Patients experiencing trouble with problems in the following areas:

1. Performing activities of daily living:
• feeding him/herself
• dressing him/herself
• bathing him/herself
• toileting him/herself
• grooming him/herself
2. moving about in bed
3. getting in and out of bed
4. limitations in arm and hand use
5. propelling him/herself in a wheelchair
6. wheelchair positioning
7. impaired thinking due to dementia and/or neurologic impairment preparing to return home after an acute illness or injury
