Rehabilitation Agency Program

If your facility is not Medicare certified, we can still help.  Through our private practices, we are able to provide rehab agencies and our therapy for your residents.  This service is available for facilities providing intermediate care, residential care, assisted living, and/or independent living.  Our highly skilled therapists provide the services, verify insurance coverage, complete all paperwork, and we do the Medicare billing.  Through our private practices, we provide you with:

• A Rehab Team led by the Key Rehab Supervisor who provides quality onsite management of your therapy program.
• Accreditation Services - staff experienced in successful state survey, CARF and JCAHO surveys will work with your staff in the preparation for survey and specialized accreditations.
• A Key Rehab Supervisor who facilitates proper documentation and assessment of residents in order to meet state guidelines.
• Professional staff round out your team.
• By adding increased on-site services, your marketing plan can be enhanced.
• Your residents are assisted to age in place due to emphasis on wellness and the ability to rapidly respond to declines.
• Ability to have baseline functional information on residents, allowing your staff to provide quality independent and personal care.
• Your personal care staff will receive education and training in wellness concepts and independent living.
• Early discharge from hospital or SNF back to your facility or organization.
• Community education by Rehab Staff.
