Key Rehab is pleased to announce that our employees can now receive a digital copy of their W2 Form. Below you can follow these easy steps to Download your W2.

• Follow the link to the log in screen
• Your login is your social security number and your date of birth
• Once you are signed in the first time, you will be directed to a consent form
• Please Click the link titled “Get My Access Code”
• This will direct you to a screen that displays your individual access code
• Enter this in the box next to the “Get My Access Code” link
• Check the box below acknowledging that this is your W-2 and you will not receive a paper copy
• Click continue at the bottom right hand side of the screen
• You are now able to view your W-2 for 2009 as well as every pay stub you received in 2009 by selecting the links to the left
• These items will be opened in pdf format and may be printed or saved
• You may log back in and view these documents through this process as many times as you like
